Authors L B Kline 1 This case report presents a patient diagnosed with Tolosa-Hunt syndrome (THS) after an extensive neuro-diagnostic and neuroimaging evaluation. Diagnostic work-up included thorough physical and neurological examination, complete laboratory serum assessments, neuroendocrine and immunohistochemistry analysis, cerebrospinal fluid analysis, neurophysiology assessment, ophthalmologist examination and neuroimaging. Tolosa-Hunt Syndrome is a rare pathological condition in which the affected individual experiences severe periorbital headaches along with ophthalmoplegia usually in one eye. The pain experienced with Tolosa-Hunt Syndrome is extremely severe and sharp and the eye movements are decreased. These symptoms in majority of the cases resolve on their own without any intervention but then again recur. Some reported cases of 13.8 Tolosa-Hunt syndrome had additional involvement of the Vth nerve (commonly the first division) or optic, VIIth or VIIIth nerves. Sympathetic innervation of the pupil is occasionally affected.
